Kara/Agent 33 first appeared in Hercules: Heart of Chaos #1 (August 1997).
Comics
Kara Lynn Palamas was a researcher and historian who has a special interest in classical mythology, particularly the Olympian god - Hercules. S.H.I.E.L.D. recruited her and she trained to become a full agent, taking the code name "Agent 33". She was partnered with Agent Alex DePaul.
Marvel Cinematic Universe
Kara Lynn Palamas first appears in the Second Season[1] of Agents of S.H.I.E.L.D., as an agent of S.H.I.E.L.D. being captured and brainwashed by HYDRA. She took orders mainly from Dr. Whitehall. After his death by Coulson, Grant Ward managed to convince her to help him. She helped him with his injuries and they fell into a romantic relationship. He tries to help gain her past back. They captured and brainwash Sunil Bakshi onto their side.
Maya Stojan portrays Agent 33; Ming-Na Wen who portrays Melinda May also portrayed Agent 33 for a few episodes when she was disguised as May, and then later when her photostatic veil was burned (although it was later fixed by the designer).
Agent 33 infiltrated S.H.I.E.L.D. which was a ruse to lure Bobbi Morse into a trap in Spain, where she and Ward captured her. According to Bakshi, Bobbi was the one who had ratted out Agent 33, which led to her brainwashing. Ward and Agent 33 tortured her in order for her to confess what she did, although Bobbi resisted. When S.H.I.E.L.D. arrived to rescue Bobbi, Agent 33 changed her face to Melinda May's to trick the S.H.I.E.L.D. agents. However, May duped her into sneaking around Ward's location, still wearing the illusion of May's face. Ward, set on revenge against May, was fooled by the illusion and shot her several times in the stomach. Agent 33 died in Ward's arms.

So far in the MCU/AOS, Kara is mainly shipped with Grant Ward (or the gen version, Kara & Ward), but she's also shipped with Jemma Simmons, Alphonso "Mack" Mackenzie, Lance Hunter, Akela Amador, Mike Peterson, Thomas Ward, and Bucky Barnes.
