This template is labeled "Survey" but the resulting infobox is called "Fan Survey" and comes with the category "Surveys by Fans." We probably need either to edit those, or to create a non-fan survey info box for academic & corporate surveys of fandom. Elf (talk) 19:55, 29 November 2016 (UTC)
