Maiden Name
Alice Longbottom has never been given a canon maiden name,[1] but fanon usually sees her as a Fortescue or, occasionally, a Prewett (sometimes spelled Prewitt).
Fortescue
Fortescue is a name which appears in canon in the character Florean Fortescue.[2] It appears as early as 2009 in the last_generation LiveJournal community,[3] 2011 on Fanfiction.net,[4] 2012 on Tumblr,[5] and 2013 on AO3.[6]
Prewett
Prewett is the maiden name of Molly Weasley, so fans who headcanon Alice as a Prewett usually regard them as cousins. Fans have used the name for her as far back as 2008.[7] Some have claimed that the name was used in JK Rowling's journal,[8] but this does appear in Frank and Alice's Pottermore article, [9] the Longbottom family tree,[10] or the journal extracts in A Journey Through a History of Magic, so the source of this claim is unknown.
