A Sam girl is a (female) fan of Supernatural who prefers the character of Sam Winchester over Dean Winchester. This fannish term was later referenced on the show itself: season 4 episode 18 "The Monster at the End of This Book".

Extreme Sam Girl is a term given to the extreme Sam stans, one fan described them as "Sam girls are devoted to Sam and will protest any perceived slights against his character by other fans, Dean, Cass, or the writers."[1]
Not to be confused with Girl Sam, AUs in which Sam is a girl.[1]
