Over-the-top (OTT) means exaggeration, melodrama, and/or excess enthusiasm[1] -- whatever that means to the speaker, in context. Simply being a fan can itself be considered OTT by non-fans.
The term OTT can be applied to canon when characters or situations go significantly beyond the expected norms, fanworks, fans, or fandom as a whole.
For example, Ranker.com displays the worst, most over-the-top Harry Potter Tattoos of All Time (#13).
The usage may be derived from the World War I term for a futile military attack against overwhelming odds [2]. The film Over The Top is a B-movie with Sylvester Stallone that illustrates both senses of the term.
